New patients should allow 45-60 minutes depending on the complexity of their case. Our office will mail you a health questionnaire a few days before your appointment. It is extremely important to bring, in addition to the questionnaire, all of your medical records for review from your referring physician, including:
- Record of history and physical exam.
- Record of all laboratory tests.
- Reports of thyroid ultrasound, biopsy, bone densitometry scans and blood sugar readings.
- The patients legal first and last name, address, date of birth, social security number and name of referring doctor.
We ask that the when a New Patient books an appointment to please provide insurance information, specifically the group number, the I.D. number and the post office address to mail claims.

Should you be a member of a Health Maintenance Organization, ie: Aetna HMO, Varsity, Health Options, please have your authorization faxed from your Primary Care Physician.
During your visit the staff will complete a history relevant to your problem and perform a physical examination. After that, the doctor will discuss your diagnosis and treatment plan and order the proper tests. After the results are back, you will receive a call and be informed of the results and what the next step will be.
If you do not receive a call within one week please call our nurse at: 587-737-7520 and ask for the results of your tests.
If you have been referred by another doctor, a letter will be sent to him or her in a reasonable period of time.
